<B>目的<B/> 近年、要介護状態になっても住み慣れた地域の中で暮らし続けることが求められている。本研究は、高齢者が要介護期にも安心して地域との関わりを持ちながら生活を続けていくために必要な地域ケアのあり方について明らかにすることを目的としている。本報では地域密着型施設を自治体主導で整備し、その施設を拠点とした高齢者の地域居住の継続に取り組んでいる加賀市の事例について報告する。<BR><B>方法<B/> 2009年2月に加賀市に対してヒアリングを行うとともに、市内の地域密着型施設に指定された施設を訪問し、地域で暮らす高齢者の生活を支援する取り組みについてヒアリング調査を実施した。<BR><B>結果<B/> 加賀市の高齢者福祉は、これまで大規模施設の整備が中心であったが、2003年に厚生労働省の高齢者介護研究会が提出した「2015年の高齢者介護」を受けて、地域密着型施設整備へと方向転換し、高齢者がこれまでの生活を継続していけるよう街中にケア施設を整備することを進めている。加賀市の施設整備の特徴として、1.市が用地選定から運営内容に関し主導的役割を担っている、2.事業者の公募の際にマニフェストを提出させ、審査も公開している、3.地域との交流を積極的に図ることを求めている、4.事業所に2ヶ月に1度、市職員や地域住民、入居者家族等を入れた運営推進会議開催を義務付けていることなどが上げられる。施設整備が成功している理由としては、1.市と事業所が理念を共有したこと、2.病院等を経営している福祉法人をバックに持つ事業所が多く小規模でも経営の心配がないこと、3.地域住民に対する説明をきちんと行っていること、などがあげられる。

<B>目的<B/> その2では、高齢者が地域との関わりを持ちながら生活を継続していくための各事業所の取り組みについて報告する。<BR><B>方法<B/> 2009年2月に加賀市の地域密着型施設の指定を受けた事業所4ヶ所を訪問し、運営者に対するヒアリング調査を実施した。<BR><B>結果<B/> 訪問した施設は、小規模多機能施設2ヶ所、グループホーム1ヶ所、小規模特養1ヶ所である。地域密着型施設は認知症の高齢者の利用がほとんどで、地域住民の認知症に対する理解が得られないと利用者の地域生活の継続は困難という認識のもと、どの事業所も地域との関係づくりのために、地域の行事に参加したり事業所の行事に地域住民を招いたりしていた。しかし、施設の種類によって地域との関係の持ち方は異なる。施設の特徴として、小規模多機能施設は日中の通いが中心で、高齢者の在宅生活支援に主眼を置いている。グループホームと小規模特養は、施設が高齢者の「住まい」であり、日常生活の安定と豊かさに主眼に置いている。そのため地域との関係は、小規模多機能施設は、施設を地域に開き地域住民が気軽に立ち寄れる場を目指しており、近隣の高齢者が昼食を食べに来る、地域のお坊さんの法話に地域住民が聴きに来るなどの事例があった。グループホームと小規模特養の場合は、入居者が街へ出かけ、出先で地域の人達と交流しており、買い物や喫茶店に出掛ける、馴染みの散髪屋へ行くなどの事例があった。また、街中立地により家族の訪問は多い。積極的に外へ出かけ地域の人と顔見知りになることで、入居者が施設を抜け出してしまった際、近所の人が見つけてくれるなど安心感にもつながっている。<BR>本研究は、平成20年度科学研究費補助金（課題番号19500645）の助成を受けて実施されたものの一端である。
